Before you come to Big Creek Surgery Center, your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. The following forms may be completed prior to your scheduled procedure.

We are contracted with Medical Mutual, CareSource, Ohio Medicaid, Medicare and Ohio Bureau of Workers Compensation.

If your insurance is not listed above you will be requested to review and sign the Patient Financial Policy below.
