Patient Registration

Before you come to Big Creek Surgery Center, your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. The following forms may be completed prior to your scheduled procedure.

We are contracted with Medical Mutual, CareSource, Anthem BC/BS, United Healthcare, Ohio Medicaid, Medicare, Ohio Bureau of Workers Compensation, and Multiplan.

If your insurance is not listed above you will be requested to review and sign the Patient Financial Policy below.
